What is Botulinum toxin and how does it work
Botulinum toxin is a substance that reversibly inhibits the transmission of signals from nerves to the facial muscles. By injecting very small amounts, specific muscles can be temporarily weakened and so-called expression lines can be reduced.
Through regular and timely application of very small amounts, Botox can prevent or at least significantly delay the development of deep expression lines. The wrinkles are unable to etch themselves into the skin in the first place.
If the wrinkles are already so deep that they are visible even without tensing the muscles in question, further improvement may be achieved after Botox treatment by combining it with plasma or hyaluronic acid.
Which wrinkles can be treated with Botox
Wrinkles caused by the tension of the so-called facial muscles are best treated. It is most commonly used in the upper facial area, but there are also several other indications:
- Frown lines
- Forehead lines
- Crow's feet, which are wrinkles at the sides of the eyes
- Diagonal wrinkles on the bridge of the nose, the so-called bunny lines
- Dimpling of the chin area (cobblestone chin)
- Contouring the jawline – Nefertiti Lift
- Drooping corners of the mouth
- hervorstehende Muskelstränge im Halsbereich (Platysma)
Botulinum toxin is also used to treat excessive sweating.
Botox treatment is less suitable for wrinkles caused by volume loss (for example, so-called nasolabial folds) or those caused by sagging skin, such as fine crinkle lines on the cheeks. In these cases, better results can sometimes be achieved for patients with hyaluronic acid, vampire lifting, PRF, or thread lifting than with Botox. Furthermore, expression lines that are already very deeply etched cannot be made to disappear with Botox alone.
How does a treatment with botulinum toxin work in my office in Vienna
First, in a detailed consultation, your ideas and wishes will be discussed in detail. On the other hand, the specific anatomy of your face and your personal facial expressions will be analyzed in detail.
Afterward, photos of the regions to be treated are taken. This is followed by a cleansing of the skin. Then, the corresponding botulinum units are injected into the facial muscles using a syringe with an extremely thin needle. Finally, the skin is briefly cooled.
The injection itself takes only a few minutes; the entire appointment, including preparation, takes 15 to 30 minutes. Above all, plan sufficient time for the prior consultation to ensure you can ask and discuss everything.

How much does a Botox treatment cost in my office in Vienna
Prices for treatment with botulinum toxin vary depending on how many regions are treated and how many units are required.
- One region (for example, frown lines or crow's feet): from €200
- Two regions: from €360
The final price depends on the amount actually used. The costs will be discussed with you in detail in advance. A detailed individual consultation is, of course, included in the price.
How long does the effect of Botox last
The effect begins to become visible about 5 days after application; the final result is achieved after about 2 weeks. It then lasts between 4 and 6 months, although there are individual differences from patient to patient.
If you want a permanent result, the treatment should be repeated about two to three times per year.
What do you need to consider after a treatment with botulinum toxin
After the treatment, minor redness or hematomas may rarely occur at the injection sites, but these side effects subside quickly. Patients rarely report temporary headaches.
For 10 days after the treatment, it is recommended to avoid sports, sauna, solarium, or vigorous rubbing of the face to prevent the injected substance from diffusing into muscles where it is not intended by the doctor.
Furthermore, no cosmetic facial treatments such as deep cleansing, lymphatic drainage, or facial massages should be performed, and home facial massage devices such as cleansing brushes, face rollers, or Gua Sha should not be used. It is best to touch the face only carefully and gently. Applying cream, washing, applying makeup, and removing makeup is allowed, but carefully and gently. Please also do not wear ski or swimming goggles in the days following the Botox treatment.
The exact procedure, possibilities, risks, and side effects or any alternatives to Botox treatment will be discussed with you in a detailed consultation at my office in Vienna.
Is Botox dangerous
Strictly speaking, Botulinum toxin A (which is the medically correct name for the substance; Botox is one of the best-known brand manufacturers) is one of the strongest known toxins. It is a neurotoxin produced by a bacterium called Clostridium botulinum. However, the active ingredient is also one of the most thoroughly researched substances in medicine.
Botulinum toxin has been used for many years in much higher quantities by neurologists as a medication to treat spastic conditions and is even approved for children in this indication. The dosages required for wrinkle treatment with Botox are many times lower.
In the skilled hands of experts, provided that the practitioner is appropriately trained and knows the correct dosage and injection technique, treatments with botulinum toxin are an excellent means of treating expression lines.
What is the perfect Botox dose?
With the chosen dosage, it is generally recommended to maintain a certain residual activity of the treated facial muscles in order to avoid a mask-like appearance. The goal is to achieve a natural and relaxed expression in which no one suspects that a doctor is responsible for the result.
When should Botulinum toxin not be used
- During pregnancy
- Breastfeeding
- Certain rare muscle diseases
- Treatment with certain antibiotics
- Acute infections in the skin areas to be treated
- With ongoing therapy using blood-thinning medications, the treatment can be performed, but there is an increased likelihood of bruising.
Although no cases have been described in which Botox has caused damage to unborn children, Botox is not approved for wrinkle treatment during pregnancy and breastfeeding and should not be used in order to avoid any unnecessary risk to the health of mother and child.
What can go wrong with a Botulinum toxin treatment
To achieve beautiful and safe results, it is absolutely recommended to visit an experienced practitioner. Minor bruising may occur during the treatment.
Rigid facial expression
If too much Botox is used, there is a risk of an unnaturally rigid, mask-like face that shows no facial expressions. It is better, especially the first time, not to use too much so that some residual facial expression is definitely preserved. You should look rested, not completely immobile. Since an ‘excess’ of Botox cannot be neutralized, it is better to perform small corrections in a second session after about 14 days than to inject too high a dose in the first treatment. (It’s similar to cutting hair – you can always make it shorter…)
Unintended asymmetries
Even more unpleasant than too high a dose is if the Botox reaches the wrong muscle group. This happens, for example, if the exact facial anatomy is not taken into account. This can block muscle groups whose mobility you actually wanted to preserve. The consequences can be asymmetries such as a crooked smile or a drooping eyelid and are extremely unpleasant for those affected. To avoid this, it is also very important that no other cosmetic treatments such as lymphatic drainage or facial massages are performed in the relevant facial area for 10 days after the Botox treatment, and heat should also be avoided.
In addition, it should be mentioned that, on the other hand, it is also possible to at least partially improve any existing asymmetries of the face through skillful treatment.
Your doctor for Botox in Vienna: Dr. Vjara Ilieva
Dr. Ilieva has specialized in Botox for years. She holds the certificate for Aesthetic Treatments with Botulinum Toxin from the Austrian Society for Dermatosurgery and has personally performed more than 3,000 Botox treatments in her offices.
Be sure to choose a doctor who is an expert, has sufficient experience and also takes the time to inform you in detail and answer all your questions.
The good thing about Botox is that although the side effects will disappear after 6 months at the latest (this is the maximum duration of action of botulinum toxin), six months is a long time if you are not happy with the result.
My Patients' Experiences
"With Dr. Ilieva, you still look natural after Botox treatment, just better. A friend asked me if I had changed something about my skincare routine."
"My sister said I looked somehow fresher, but couldn't say why. That's exactly what I wanted."
"My face looks much more relaxed, but I still look like myself. No one has suspected Botox is behind it."
"I didn't want to look different, but to recognize myself again, but look less tired and tense. That's what I achieved."
"I look like myself, just more rested. My friends and family notice the change, but don't recognize the cause."
"I had the treatment for myself. The fact that others now perceive me as more rested is a pleasant side effect."
"It doesn't feel like a change, but like a more harmonious facial expression."
"I look like myself, just without that permanent strained expression."
"I don't get compliments for a treatment, but for my appearance. That says it all, really."
What alternatives are there to Botox
When it comes to treating so-called expression lines, Botox is clearly the best alternative because it literally eliminates the cause of the problem.
However, I also offer a variety of innovative and effective alternatives to Botox to reduce wrinkles and let your skin glow youthfully – both in case the wrinkles that bother you are not suitable for Botox, and in case you simply do not want Botox treatment.
A popular method is hyaluronic acid filler treatment, in which natural hyaluronic acid is injected into the skin to restore volume and smooth wrinkles. Hyaluronic acid fillers are particularly suitable for deep wrinkles and contour improvement.
A light version of hyaluronic acid treatment includes skin boosters and Profhilo. Here, it is not so much about adding volume, but rather increasing the moisture content of the skin, which provides more glow.
Microneedling is another effective alternative. This involves using tiny needles to stimulate the skin and trigger natural collagen production. If the treatment is additionally combined with PRP, also known as vampire lifting, the regenerative effect is significantly enhanced.
Chemical peels are another option that can improve the complexion and reduce wrinkles by removing the upper layers of skin.
I would be happy to help you find the right treatment for your needs and skin texture.
Other applications: Lip Flip, Nefertiti Lift, and Gummy Smile
What is a Botox Lip Flip?
Here, small amounts of Botox are injected into the upper lip muscles. This relaxes them and makes the lip appear slightly fuller. Additionally, it prevents the upper lip from rolling inward when laughing. It is important to note that the result depends on the individual lip shape and the method is not suitable for everyone. This procedure should also only be performed by experienced doctors, as it can lead to very unpleasant side effects if applied incorrectly.
What is a Nefertiti Lift?
The name comes from the Egyptian Queen Nefertiti, who was considered one of the most beautiful women of antiquity. Her famous bust shows a beautiful queen with a perfect jaw and neck line. In the Nefertiti Lift, botulinum toxin is used to tighten and rejuvenate the appearance of the neck and lower jaw.
What is a Gummy Smile?
Gummy Smile is when too much of the gums of the upper jaw is visible when laughing. A special treatment with botulinum toxin can make the smile a little more harmonious here.
Frequently Asked Questions about Botox in Vienna (FAQ)
What is the difference between fillers and Botox?
Botox is a medication that inhibits muscle movement. Thus, Botox is used to treat expression lines, such as frown lines or forehead wrinkles. Fillers, on the other hand, are substances used to give volume to the face. Hyaluronic acid fillers are used, for example, to restore lost volume in the midface or to plump up the lips.
Is Botox treatment painful?
Of course, the perception of pain is always individual for each patient, but overall, while a Botox treatment is slightly uncomfortable, it is not particularly painful. Most people only feel a slight stinging sensation. Extremely thin needles are used. Additionally, the affected area can be made less sensitive through cooling or small devices that send vibration pulses. Furthermore, the treatment really does not take very long.
What happens during the Botox treatment?
First, a detailed consultation takes place in my practice. Then, photos of the areas to be treated are taken. Afterwards, the skin is cleaned and the Botox is injected under the skin with thin needles. Finally, the skin is cooled. A follow-up appointment after 14 days, during which small adjustments can be made if necessary, is included in the price.
Do existing wrinkles disappear completely with Botox treatment?
By relaxing the facial muscles, the treated wrinkle can no longer be made. Unfortunately, deep wrinkles that are already heavily etched into the skin do not disappear completely as a result. However, they are softened through regular treatments.
At what age should one start with Botox?
How early expression lines form is very individual and depends on genetics, lifestyle, and the intensity of facial expressions. There is no general answer to this question. However, the best time, if you want to prevent the development of deep wrinkles, is when a wrinkle is just beginning to show as a fine line even when the face is relaxed.
Do men also get Botox injections?
Of course, men can also be treated with botulinum toxin. Since certain facial features are generally perceived as more masculine or more feminine, slightly different dosages of the toxin and slightly modified injection points are often chosen for men. Experienced doctors can achieve the optimal result here, taking the patient’s wishes into account, without it being immediately obvious that a treatment has been performed.
Are my wrinkles worse when I stop using Botox again?
No, wrinkles do not get worse if you stop using Botox. When the effect has worn off after 4 to 6 months, the muscles simply return to their original level of activity, and you look as you did before the treatment. However, it does happen that patients get used to the effect and it seems to them as if the wrinkles are worse afterward than they were.
What is the Botox effect?
The term ‘Botox effect’ is used when Botox is applied repeatedly in the same facial region. The muscles in question can become weaker through repeated immobilization, which can make the result last longer.
Are there any long-term effects with Botox?
Botox is inherently a neurotoxin, and that naturally sounds quite scary at first. However, many long-term studies have shown that Botox does not cause long-term consequences and does not affect your health. The effect completely regresses after six months. However, muscles that are inactive for a long time lose their strength, similar to the muscles in arms and legs when we wear a cast for a long time. It is no different with facial muscles; these can also become weaker through long-term immobilization.
Is all Botox the same?
In Austria, several preparations from different manufacturers are approved. Which one is used is usually the doctor’s personal preference. However, you should definitely not order botulinum toxin yourself on dubious sites on the Internet, as there is no guarantee what you will actually get.
How many units do I need?
How many units you need for wrinkle treatment varies individually. On the one hand, it depends on how strong your muscles are, and on the other hand, on how much residual movement you would still like to have. While it is common in the Anglo-American region to actually sell the toxin by units, in Germany and Austria it is common to state the costs for a region, even if there is a certain range in the required units. Furthermore, there are several different providers of botulinum toxin preparations in Austria, between which conversion factors exist: one unit of toxin from one company does not correspond to one unit of toxin from another manufacturer.
How much does a treatment with Botox cost in Vienna?
Prices for Botox treatment vary depending on how many regions are treated and how many units are required. When treating one region (for example, frown lines or crow’s feet), costs from €200 can be expected. The combined treatment of frown lines and forehead wrinkles costs €360. The costs will be discussed with you in detail beforehand.
Can a Botox treatment be combined with other treatments?
Of course, you can also combine Botox treatment with other treatments, such as fillers, PRP, skin boosters, Profhilo, chemical peels or microneedling. This can even produce particularly beautiful results. However, it is advisable to spread these treatments over two separate appointments if the same area of the face is being treated, for example crow’s feet or upper lip lines.
Can one develop antibodies against Botox?
In rare cases, patients develop antibodies against Botox, and the preparation therefore no longer works or only works weakly. This occurs more frequently when Botox is used in neurology because much larger quantities of the substance are used there than in aesthetic medicine. To avoid this nonetheless, there should always be at least three to four months between individual treatments. This means it is better to treat all regions you want to address at once, rather than the frown lines one month, the crow’s feet the next, etc. Should there still be a suspicion that Botox is no longer working for you, you can take a longer break so that the body can break down the antibodies again, or switch to a different botulinum toxin preparation.
